ANNUAL DISCLOSURE

I hereby acknowledge that I read, had the opportunity to ask for any clarifications I deemed necessary, and understand the definitions and other provisions of the Henry Ford College Board of Trustees Conflict of Interest, Conflict of Commitment, Nepotism, and Outside Activities Policy ("Policy").
I understand that while serving Henry Ford College, I should not be personally involved in any arrangement that could create a direct, indirect, apparent, actual or possible conflict of interest or commitment with the College. I further understand that conflicts of interest are those personal and financial interests, whether actual, apparent or possible, that could lead me to: (a) compromise or lose my own independence, impartiality or judgment in connection with an arrangement with the College; (b) propose or support an arrangement with the College that is not in the College's best interest; (c) results in personal or financial gain to me; (d) involves preferential treatment to my family or business associates; or (e) would damage the College's reputation or erode the public's confidence in the College. The Policy contains the following definitions:
(a) A “personal relationship” is considered a relationship between individuals who have, or had, a continuing relationship of a romantic or intimate nature. Personal relationships between faculty and students enrolled in the faculty’s course are prohibited.
(b) Gifts. An Employee or Trustee may not solicit or accept, directly or indirectly, any gift, gratuity, favor, entertainment, or loan; opportunity to purchase an item at cost, discount, or bargain price, unless such opportunity is available to the general public; or other thing of monetary value from a person or entity which:
- Has or is seeking to obtain, contractual or other business or financial relations with the HFC or the Dearborn Public Schools;
- Conducts operations or activities that are affected by the HFC or the Dearborn Public Schools; or
- Has interests that may be substantially affected by the performance or non-performance of the employee's official duty or by a Trustee's vote or efforts to persuade other Trustees to vote in a given manner.
Employees or Trustees may accept nominal gifts, entertainment, and food when it is clear that it is solely for family or personal relationships and not to influence a decision maker. Employees or Trustees may accept non-solicited advertising or promotional material, such as pens, pencils, note pads, calendars, and other items of nominal intrinsic value.
(c) “Relative” is defined as a member of the employee’s immediate family which includes spouse, parent, child, grandparent, sibling, grandchild, brother-in-law, sister-in-law, son-in-law, daughter-in-law, mother-in-law, father-in-law, step-parent, step-child, domestic partner, or legal guardian or other person who stands in the place of a parent. An employee who is related to, or involved in a personal relationship with another employee may not work directly for or supervise that employee.
Pursuant to the Policy I make this annual disclosure:
1. I have no personal, financial, or business interest that could involve an arrangement with the College.
2. None of my family or business associates have a personal, financial, or business interest that could involve an arrangement with the College.
